Takming University of Science and Technology Online Application Agreement
一、本人已詳閱並同意招生簡章相關規定。
I have read and agree to the relevant regulations outlined in the admission brochure.
二、網路報名各項資料均由本人或授權他人輸入，報名表輸入資料或附繳各項文件確為本人所有，倘經發現與事實不符或查驗之學歷（力）等證明文件不符，本人願負法律責任並同意被取消錄取及入學資格。
All information submitted to the online application has been provided by me or an authorized representative. I confirm that the details in the application form and the accompanying documents are accurate and my own. If any information is found to be false, or if the academic credentials provided do not match the verified records, I accept full legal responsibility and agree to the cancellation of my admission and enrollment eligibility.
三、本人同意報名所填各項資料，由貴校依據「個人資料保護法」等相關法令規定，為必要之資訊應用，處理相關事宜。
I agree that the information provided on my application forms may be used by the university for necessary purposes in accordance with the Personal Data Protection Act and other relevant regulations.

Takming University of Science and Technology Notice Regarding the Collection, Processing, and Use of Applicants' Personal Data
依據「個人資料保護法」（以下簡稱個資法），為確保您的個人資料，隱私及權益之保護，請詳細閱讀德明財經科技大學（以下簡稱本校）依個資法第8條及第9條規定所為以下「考生個人資料蒐集、處理及利用告知事項」。
In accordance with the Personal Data Protection Act (hereinafter referred to as and to ensure the protection of your personal data, privacy, and rights, please carefully read the following "Notice on the Collection, Processing, and Use of Applicants' Personal Data" provided by Takming University of Science and Technology (hereinafter referred to as "the University") in compliance with Articles 8 and 9 of the Act.
一、機構名稱：德明財經科技大學。
Name of Organization : Takming University of Science and Technology
二、個人資料蒐集之目的：基於辦理本校招生入學考試相關之試務、成績、錄取、報到、查驗等，資(通)訊與資料庫管理、統計研究分析、註冊入學後之學生資料管理及相關或必要工作之目的。
Purpose for the Collection of Personal Data:The administration of the university’s admission examinations involves a range of responsibilities, including exam procedures, grading, admissions, enrollment, verification, information and database management, statistical research and analysis, post-enrollment student record management, and other related works.
三、個人資料蒐集之方式：透過考生親送、郵遞、傳真或網路報名而取得的考生個人資料。
Methods of Personal Data Collection:Personal Data of the applicants are collected through personal submission, post mail, fax, or online application.
四、個人資料蒐集之類別：考生姓名、身分證統一編號或居留證或護照號碼、出生年月日、相片、性別、報考資格、緊急聯絡人、住址、E-mail帳號、聯絡電話、手機、低收入戶或中低收入戶證明方式、轉帳帳戶等。法定個人資料類別(註)為：C001、C002、C003、C011、C021、C023、C033、C034、C038、C051、C052、C056、C057、C061、C062、C064、C072、C111、C132。
Categories of Collected Personal Data:Applicant’s name, National ID number, residence permit or passport number, date of birth, photo, gender, eligibility for application, emergency contact person, address, email account, contact number, mobile number, proof of low-income or middle-low-income status, bank transfer account, etc.
The corresponding legal personal data categories (as per the Personal Data Protection Act) are:
C001, C002, C003, C011, C021, C023, C033, C034, C038, C051, C052, C056, C057, C061, C062, C064, C072, C111, C132.
五、個人資料處理及利用：
Processing and Use of Personal Data:
(一)個人資料利用之期間：除法令或中央事業主管單位另有規定辦理考試個人資料保存期限外，以上開蒐集目的完成所需之期間為利用期間。
Duration of Personal Data Use: Unless specified by laws or regulations established by the central competent authority, the data will be utilized for the necessary duration to achieve the purposes of collection.
(二)個人資料利用之地區：台灣地區（包括澎湖、金門及馬祖等地區）或經考生授權處理、利用之地區。
Geographic Area for the Use of Personal Data includes Taiwan, which encompasses Penghu, Kinmen, and Matsu, as well as any other regions authorized by the applicants for processing and utilization.
(三)個人資料利用之對象：本於完成招生試務蒐集目的之本校各單位、教育部及其他法定主管機關。申請特殊應考服務考生之健康紀錄及應考人資料，僅供本校提供應考服務之依據，不作為其他用途。
Recipients of Personal Data Usage: The collected data will be utilized by relevant university departments, the Ministry of Education, and other legally authorized regulatory agencies to fulfill admission-related purposes. Health records and applicant information for individuals requesting special examination accommodations will be used solely to provide necessary services and will not be applied for other purposes.
六、個人資料利用之方式：本校進行試務、成績、錄取、報到、查驗、註冊、入學生管理等作業，考生（或家長、監護人）之聯絡，基於試務公信的必要揭露（榜示）與學術研究及其他有助上開蒐集目的完成之必要方式。
本校得依法令規定或主管機關或司法機關依法所為之要求，將個人資料或相關資料提供予相關主管機關或司法機關。
當事人如未提供真實且正確完整之個人資料，將導致無法進行考試報名、緊急事件無法聯繫、考試成績無法送達等，影響考生考試、後續試務與接受考試服務之權益。考生應確認提供之個人資料，均為真實且正確；如有不實或需變更者，應即檢附相關證明文件送交本校國際事務處辦理更正。
The collected personal data will be utilized for examination administration, grading, admissions, enrollment, verification, student management, and communication with applicants or their parents/guardians. Additionally, it may be disclosed when it is necessary to maintain public trust in the examination process, for academic research, or for other essential purposes related to the stated objectives.
The university may disclose personal data or related information to relevant authorities or judicial agencies as required by law or upon request from regulatory or legal bodies.
If applicants fail to provide accurate and complete personal information, it may result in an inability to register for the examination, receive notifications in case of emergencies, or obtain examination results, thereby affecting their rights in the admission process and related services. Applicants must ensure that all submitted personal information is accurate and truthful. If any information is incorrect or requires changes, they must submit the relevant supporting documents to the university’s Office of International Affairs for correction.
七、考生得依個資法規定查詢或請求閱覽；請求製給複製本；請求補充或更正；請求停止蒐集、處理或利用；請求刪除。考生得以書面、傳真、電話等方式與本校國際事務處聯絡，行使上述之權利。惟停止蒐集、處理、利用或請求刪除個人資料之請求，不得妨礙本校依法所負之義務。
Applicants have the right, by the **Personal Data Protection Act**, to:
- Request to **inquire about or review** their personal data.
- Request a **copy** of their personal data.
- Request to **supplement or correct** their personal data.
- Request to **stop the collection, processing, or use** of their data.
- Request the **deletion** of their personal data.
Applicants may exercise these rights by contacting the university’s Office of International Affairs by written request, fax, or phone. However, requests to stop the collection, processing, use, or deletion of personal data must not interfere with the university’s legal obligations.
